The Michigan Department of Education, Low-Incidence Outreach (MDE-LIO) is a trustee for the American Printing House for the Blind (APH) in Michigan. As such, they are tasked with providing eligible students with visual impairments with accessible instructional products. Initially, MDE-LIO accepted requests for such products via a basic online form. The Center for Educational Networking (CEN) was asked to revisit the order form and make it more usable.

After analyzing the the existing form, I concluded that making it usable required much more functionality than a basic web form could provide. I designed a set of mockups that were approved by MDE-LIO after a few revisions. After approval, I developed a ‘skeleton’ in code that provided the basic functionality using HTML, CSS, and jQuery and handed it off to CEN’s backend developer.
